The cash rates shown above are based on current Ohio Medicaid Community Health Center reimbursement rates.

All providers are paneled with Ohio Medicaid. Currently only Alvin Mares and Patricia Mares are paneled with most major commercial insurance payers (e.g., Anthem, Optum, Aetna, Medical Mutual). However, neither Alvin nor Patricia are accepting new patients. So ours is primarily a Medicaid + Fee For Service practice.

 

The out-of-pocket cost for new clients/patients to see Licensed/MA and Mental Health Specialist providers is the cash rate shown above. The cost to see an Independently Licensed provider will include any applicable deductible, copayment and/or co-insurance fees determined by the insurance payer for an in-network provider.

Superbills are available upon request for patients with private/commercial insurance paying cash to meet with out-of-network Licensed and/or Peer provider(s) who wish to submit claims for reimbursement from their insurance company of the cash rate paid to Mares Cares.

Applicable out-of-pocket fees for service are due (collected) at the time of service. Credit or debit card (American Express, Discover, MasterCard, or Visa) is the preferred form of payment, with cash and check also accepted.

Unlike other services which may be billed or reimbursed by third party payers, Parenting Coordination may only be paid by parties directly, most often with the cost shared equally (50/50) between birth parents. For this reason and because it is most often ordered by the Court, a $1,500 retainer ($750 paid by each party) is collected before services begin (check payable to "Mares Cares Counseling", with "PC retainer" written on memo line).
