The cash rates shown above are based on current Medicare reimbursement rates for 2022, for the State of Ohio, and for the five-digit current procedural terminology (CPT) codes indicated.
Actual out-of-pocket cost ("copay") may be less than the cash rate, depending on two factors: 1) insurance eligibility and coverage; and, 2) household size and household income of the client. Insurance companies negotiate a discounted rate (aka, "allowed amount") for counseling services provided by in-network providers.
Some providers are paneled "in-network" with various private (employer-based) payers, such as Anthem, Medical Mutual, UnitedHealthcare/Optum, and/or public payers, such as Medicaid, CareSource, Molina, Medicare, Tricare. Those who are not paneled may see clients/patients on a cash (fee-for-service) basis.
Clients living in households earning less than three times the federal poverty level qualify to receive a discounted "sliding fee scale" rate. This income-based discount may be applied to these cash rates.
Copays are due (collected) at the time of service. Credit or debit card (American Express, Discover, MasterCard, or Visa) is the preferred form of payment, with cash and check also accepted. Special payment arrangements are also available.
